There is only one form to accomplish for the REC Clearance Application.

FORM 1 Protocol Submission and Receipt Form

Submit filled out forms with complete attachments to the MSUM REC Office Secretariat at the OVCRED Office (2nd Floor, Old Admin Building). Only complete documents are accepted and will be considered for processing. Send soft copies to research.ethics@msumain.edu.ph.
